This form is for submitted any safety concerns you are aware of within the Deschutes Flying Club or the operation of its aircraft and members.
This form can be submitted anonymously, or optionally, you can include your name or contact information in your report if you choose.
This safety concern will be promptly brought to the attention of the Board of Directors and changes in policy, procedure, etc. will be implemented as needed.
Please include as much information as possible, what occurred, what risks it showed, and what your proposed solutions or what changes should be made, etc.
